PPN: Invoicing Process

This document outlines the end-to-end process for 3X4 Genetics contracted Practitioners to:

1. Complete vendor onboarding via the Ramp platform, and
2. Submit accurate monthly invoices for Practitioner consultations performed through the Healthie platform.

⚠️ HIPAA: All billing must comply with HIPAA requirements. Client names must never appear in any billing documentation.

Process Overview

The 3X4 Genetics Practitioner billing process involves three phases:

Phase 1: Vendor Onboarding (One-Time Setup)

Before submitting any invoices, all Practitioners must complete vendor onboarding through the Ramp platform. This is a one-time process that collects your tax information and banking details so 3X4 Genetics can process ACH payments directly to your bank account.

📝 Any changes to banking details will be made by the Practitioner in Ramp to avoid fraud.

Step 1: Accept Your Ramp Vendor Invitation

3X4 Genetics will initiate your onboarding by sending a secure, automated invitation email directly from Ramp on behalf of 3X4 Genetics. When you receive this email:

• Check your inbox for an email from Ramp on behalf of 3X4 Genetics.
• Click the secure vendor portal link provided in the email.
• Do not forward or share this link. It is unique to your account.

Step 2: Submit Your Tax Information (W-9)

Through the Ramp vendor portal, you will be prompted to submit your tax classification and identification. This fulfills the IRS Form W-9 requirement.

Step 3: Submit Your Banking Information (ACH)

To receive direct deposit payments, you must submit your banking details through the Ramp portal. This fulfills the ACH Authorization requirement.

Required banking information:

• Bank name
• Bank address
• Account name (as it appears on your bank account)
• Bank account number
• ABA routing number
• Account type: Checking or Savings

What Happens Next?

1. You will be able to submit your invoices directly through Ramp.
2. You will be able to check the status of your payments.
3. You may opt to join the Ramp Vendor Network, a way for payers and vendors to collaborate more easily and securely on Ramp. Payers can get updates from vendors that have an existing Vendor Network profile and use their stored payment and tax information, eliminating the need for manual entry and sharing via less secure channels.

Phase 2: Performing Consultations on the Healthie Platform

3X4 Genetics offers a bundled product in which clients purchase a genetic testing kit along with a Practitioner consultation. Consultations are performed through the Healthie platform.

Step 4: Log In and Access Your Healthie Account

1. Log in to your Healthie practitioner account using your assigned credentials.
2. Navigate to your scheduled appointments for the billing period.

Step 5: Record Consultation Details for Billing

For every scheduled session, record the following information at the time of the appointment. You will need this to complete your monthly invoice.

• Timestamp: Date and time of the consultation (e.g., 2026-05-14 at 2:00 PM EST)
• Session Outcome: Occurred | Late Cancellation | No-Show

Session Outcome Definitions

• Occurred: The practitioner and client both attended the session in full.
• Late Cancellation: The client cancelled the appointment with less than 24 hours' notice.
• No-Show: The client did not attend and did not cancel in advance.

Phase 3: Monthly Invoice Submission

Practitioners must submit a completed invoice to 3X4 Genetics by the 5th calendar day of each month, covering all sessions performed in the prior calendar month.

Step 6: Complete the 3X4 Genetics Practitioner Invoice Template

Use the official 3X4 Genetics Practitioner Invoice Template provided by 3X4 Genetics. Forward it to 3x4genetics@ap.ramp.com or upload it directly through the Ramp Vendor portal.

⚠️ Remember: client names must never appear on your invoice or in any billing documentation.
